Urology Department at Indraprastha Apollo Hospitals Delhi, India

The services provided encompass:

Comprehensive care for the urinary stone diseases - Stone disease is a very common problem all over the world. Stones can either be managed by lithotripsy and or may need various endourological procedures to remove the stones without open surgery.

Lithotripsy - A method of treating kidney and ureteric stones without surgery, using the latest Dornier lithotriptor-Delta model. One of the best machines available for the purpose. Both x-ray & Ultrasound controls are available on this machine. The positioning, focussing and delivery of shock waves are all controlled by advanced computerized system with safety cut-offs to prevent damage by human error. Patients does not need any hospital stay and can go home same day.

Percutaneous renal surgery (PCM) - Here 1 cm hole is made in kidney to remove stones after breaking it into smaller pieces. There is a minimal pain and discomfort and patients can go home in 48 hours. Even bigger stones and stag horn stones can be safely managed with this modality. Some of the patients may also need help of lithotripsy to get complete stone removal.

Intra-renal stone surgery by flexible scopes and laser lithotripsy - Here flexible scopes are passed through the urethra (from where one passes urine) and stones are removed after breaking into smaller pieces by laser. Here again patients can be discharged within 24 to 48 hours.

Endourology - All aspects of Endourology (surgery through a telescope, which does not require open procedures) including treatment of prostate (TURP), bladder cancers (TURBT), urethral strictures, kidney and ureteric stones (PCNL & URS) are available. Intracorporeal Lithotripsy (breaking of stones using endoscopes) using the pneumatic lithotripsy is also available.

Minimally Invasive surgery (Keyhole or laparoscopic surgery) - Most of the urological surgery can be performed by keyhole surgery. Most of kidney surgery whether it is for benign disease or for cancer can be safely performed laparoscopically. In the same way many other urological cancer surgery like prostate cancer or ureteric cancer can also be done without cutting the abdomen. Apollo hospital offers all kinds of laparoscopic surgery for urological disease. Major advantage is less pain, small scar or no scar, early discharge and early return to work.

Laparoscopic Donor nephrectomy - Now donor can donate one of the kidney to their loved on without much pain and discomfort. Even kidney retrieval incision is not seen and donor can go home in two to three days.

Laser for prostatic surgery - Men after the age of 50 years suffer from urinary problems like going to toilet frequently, poor control of urine or difficulty of passing urine etc. It is because of enlargement of the prostate, which can be removed safely by endoscopic method. Apollo hospital now has facility to remove these glands by laser surgery.

Urodynamics - The most advanced fully computerised urodynamic machines at Apollo, which will diagnose all kinds of incontinence and difficulty in passing urine so as to enable the right treatment. These machines also include 'EMG' facilities that are a must for diagnosing urinary and bowel problems secondary to spinal injuries stroke, parkinsonism and other neurological disorders. Founded in 1996 (January), and recognized as one of the major cardiothoracic surgery units of North India, this division has contributed over 20,000 cardiac surgeries encompassing the entire spectrum of surgical treatment of heart diseases, be it coronary artery bypass, intracardiac repairs of congenital heart defects, valvular heart disease or aortic aneurysm repairs.

Cardiac Surgery Consultants and Cardiac Anaesthesia Consultant are available round the clock. Three fully functional operating suites equipped with L. Spectrum of Services Offered

 

A. Coronary Artery Bypass Graft Surgery: The department was one of the pioneers of beating heart surgeries in 1999 and since 2000; this method continues to be the procedure of choice of surgical revascularisation of coronary artery disease.

Even though, this procedure does not require cardiopulmonary bypass, all the three operating suites are equipped with heart lung machine and Intra-aortic balloon pumps. The homolougus blood transfusions are minimal and can be avoided totally in large number of cases when cell saver is used.

In addition to primary CABGs, the centre is known for its proficiency in REDO CABG and combined procedures like AVR + CABG, MVR + CABG and carotid endarectomy. The internal mammary arteries (Left/Right/Bilateral) are the most preferred conduits for CABG except Bilateral IMA is avoided in insulin dependent in diabetics.

The radial artery is the next best conduit. Aorto coronary by-pass is the usual mode of surgical revescularisation, but LIMA – RIMA Y graft is often used in younger patients for Total Arterial Revascularisation.

Acute and urgent surgical intervention maybe required to treat acute coronary syndrome complicated by left main stenosis, Post infarction VSD, Impending Cardiac Rupture or Acute Mitral Regurgitation and the unit gears up 24/7 to envisage these lifesaving operations with good outcomes.
Valuable adjuncts like a large number of ventilators and Intracardiac balloon pumps help in salvaging these very sick patients. Redo CABG are readily accepted and large numbers have been carried out with 95% success.

B. Valve Surgery: All valve surgeries are undertaken with confidence and both repairs and replacements are done with good outcomes. Both Prosthetic (metallic) valve substitutes and Bio-prosthetic (tissue) valve substitutes are available. The surgeons are fairly experienced in carrying out valves repairs whenever possible and endeavor to preserve the subvalvar apparatus during mitral valve replacements. Intra –operative Transesophagial echocardiography is available whenever needed. 

C. Aneurysm Surgery: Aortic aneurysms are handled with confidence. Elective and Emergency surgeries are undertaken to take care of aortic dissections day or night. A number of aortic root replacements (Bentall operations) have been done with 90-95% success. 

D. Congenital Heart Defects: The entire gamut of congenital heart defects are accepted for repairs. Neonatal arterial switch operations for Transposition are done regularly. Other intra cardiac palliative and corrective surgeries are offered for complex congenital defects. Epicardial chocardiography is available for assessing efficiency of procedures carried out. 

E. Thoracic Surgery: All thoracic surgical procedures like lung resections decortications, volume reduction surgery are undertaken by dedicated team of Thoracic Surgeons. VATS (Video Assisted Thoracoscopic Surgery) is also available. 

F. Vascular Surgery: Peripheral vascular disease, abdominal aortic aneurysm and carotid artery disease are appropriately handled by dedicated surgeons when indicated.

Patient Endoscopy in Indraprastha Apollo Hospitals Delhi, India

Endoscopy is routinely performed at the Centers for Medical and Surgical Gastroenterology, Apollo Hospitals, India for the following conditions:

For patients who suffer from gastrointestinal bleeding, both Banding / injection of esophageal varices and control of bleeding with adrenaline injection / argon plasma coagulation are routinely performed.

·         For patients with swallowing difficulty due to esophageal stricture or achalasia, endoscopic dilatation can be carried out under fluoroscopic guidance. Similarly dilatation of pyloric and colonic strictures can also be carried out. In patients with Esophageal tumours, metallic stent placement is done, after dilatation.
·         For patients for whom feeding is a problem either due to neurological disorders or altered consciousness levels, endoscopic placement of nasojejunaltube or PEG (Percutaneous Endoscopic Gastrostomy) tube is done.
·         Endoscopic removal of gastric / colonic polyps is routinely done.
·         For patients with obstructive jaundice, ERCP / papillatomy / stone extraction from CBD/biliary stenting (plastic as well as metal stents) can be performed.
·         Placement of pancreatic stents in patients with severe pain due to chronic calcific pancreatitis is done.
·         Esophageal manometry, pH studies and rectal manometry, which are available in only limited centres, are done routinely and help not only in diagnosis, but also in deciding which patients with gastroesophageal reflux or achalasia are likely to require surgery.
Other routine procedures like liver biopsy, aspiration of liver abscess and diagnostic and therapeutic paracentesis are done through endoscopy.

Q- What Is GI Endoscopy?

Ans- GI Endoscopy

Gastrointestinal endoscopy is done when to see the inside of the stomach or intestines.
Gastrointestinal endoscopy involves placing a thin tube-like instrument through the mouth or
the back passage, the rectum, and down into the gullet, or oesophagus, stomach or intestines.
The tube is able to carry pictures back to a video screen or camera. When endoscopy is done 
through the mouth, it is called upper endoscopy or esophagogastroduodenoscopy (EGD for short).
When endoscopy is done through the rectum, it is called lower endoscopy or colonoscopy.

Common reasons for its use and benefits:

  • BLEEDING - The most common reason for gastrointestinal endoscopy is to find the
  •  source of bleeding from the oesophagus, stomach or intestines. Sometimes, if the source
  •  is found, doctors can use the endoscope to stop the bleeding ? TUMOURS - Sometimes
  •  tumours can cause discomfort or even clog the oesophagus, stomach or intestines. The
  •  endoscope can be used to find tumours and take a small sample for analysis in the lab, 
  • called a biopsy.
  • DIARRHEA - Sometimes severe diarrhea can be caused by inflammation or infections
  •  of the colon and endoscopy can be used to help find the cause ? BELLY PAINS -
  •  Sometimes severe stomach pain can be a sign of ulcer, a clog in the gastrointestinal track,
  •  inflammation or infection. Endoscopy can help find the reasons for some belly pains.

Risks:

Some of the risks though rare of gastrointestinal endoscopy include:
  • A low blood pressure (called hypotension) – Frequently, medicines are given to help 
  • keep patients comfortable during endoscopy. Some patients develop brief drops in
  • the blood pressure during endoscopy. Such drops can be life-threatening and can 
  • be a reason for stopping endoscopy before it is finished.
  • oesophagus, stomach or intestines can be damaged by the endoscope and the
  • contents can leak into the surrounding area of the body. This is a serious problem
  • which can cause infection and even death.
  • Bleeding - The oesophagus, stomach or intestines can be damaged by the
  • endoscope causing some internal bleeding.
